CalAim Manager
Description: The CalAIM Manager is responsible for overseeing the planning, implementation, and daily operations of all CalAIM-aligned programs at Good News Rescue Mission, including Enhanced Care Management (ECM), Community Supports (Housing Navigation, Housing Deposits, Housing Tenancy \& Sustainability), and Short-Term Post-Hospitalization (STPH) housing services. This role ensures full compliance with Medi-Cal, Partnership HealthPlan of California, and California CalAIM requirements, while providing leadership, supervision, and strategic direction to multidisciplinary teams serving high-acuity populations. The CalAIM Manager serves as a primary liaison between internal departments, managed care plans, hospitals, county agencies, and community partners to ensure coordinated, ethical, and effective service delivery. Responsibilities: · Program Oversight \& Compliance o Oversee all CalAIM-aligned programs, including ECM, Community Supports, and STPH housing. o Ensure compliance with Medi-Cal, Partnership HealthPlan, CalAIM contracts, and applicable regulations. o Monitor service delivery, documentation, billing, and encounters to ensure accuracy, timeliness, and audit readiness. o Develop, implement, and update program policies, procedures, and workflows aligned with regulatory and funding requirements. o Serve as a subject matter expert on CalAIM services, eligibility, and operational standards. · Staff Supervision \& Leadership o Supervise, train, onboard, and support Care Managers, Housing Navigators, Outreach staff, and other program personnel. o Conduct performance evaluations, provide coaching, and implement corrective action as necessary. o Establish clear expectations for productivity, documentation, and professional conduct. o Provide guidance on complex cases, ethical issues, and client safety concerns. o Promote trauma-informed, recovery-oriented, and person-centered care practices. · Care Coordination \& Client Services o Oversee care coordination for high-acuity clients with complex medical, behavioral health, housing, and social needs. o Collaborate with hospitals, clinics, behavioral health providers, and social workers to ensure continuity of care. o Support transitions from hospitals, detox, residential treatment, and incarceration into housing and services. o Address escalated client concerns, grievances, and safety issues as needed. o Ensure appropriate referrals to medical, behavioral health, substance use treatment, and community resources. · Housing \& Facility Coordination o Oversee STPH housing operations, including eligibility determination, placements, length-of-stay tracking, and transition planning. o Coordinate with maintenance, facilities, and leadership regarding housing safety, remediation, and compliance matters. o Collaborate with Housing Navigators on placement strategies and housing retention plans. o Address operational concerns related to guest safety, health protocols, and property management. · Billing, Data \& Reporting o Oversee CalAIM billing processes, including review of services documented in RXNT. o Track program metrics, encounter targets, utilization, and outcomes. o Prepare reports for internal leadership, managed care plans, and funders. o Identify trends, gaps, and opportunities for quality improvement. · External Partnerships \& Representation o Serve as the primary point of contact for Partnership HealthPlan, county agencies, hospitals, city officials, and other stakeholders related to CalAIM services. o Participate in cross-system meetings, case conferences, and collaborative initiatives. o Support program expansion efforts, including implementation in new counties or service areas. · Risk Management \& Quality Improvement o Identify and mitigate risks related to client safety, compliance, and service delivery. o Respond to incidents, audits, and corrective action plans as required. o Lead quality improvement initiatives to enhance program effectiveness and staff sustainability. o Ensure adherence to confidentiality, HIPAA, and ethical standards. Minimum Job Requirements: · Experience working with high-need populations, including individuals experiencing homelessness, substance use disorders, mental health challenges, and complex medical needs · Strong knowledge of CalAIM, Medi-Cal managed care, care coordination, and housing-related services · Supervisory and leadership experience in human services, healthcare, or nonprofit settings · Excellent organizational, communication, and problem-solving skills · Ability to collaborate across departments and with external systems · systems, and grant compliance Preferred qualifications · Registered or Certified Substance Use Disorder Counselor (SUDRC, CADTP, or equivalent) · Experience using RXNT or similar electronic health record (EHR) systems · Experience working with Medi-Cal managed care plans and CalAIM billing Knowledge, Skills, and Abilities Required: · Strong organizational and time-management skills. · Excellent written and verbal communication skills. · Proficient in Microsoft Office and client management software. · Ability to work effectively with diverse populations in challenging environments. · Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. · Be a Team Player who is focused on the overall success of the organization. · Ability to foster a cooperative work environment. Work Environment: · Work is performed in both indoor and outdoor environments · Includes outreach-based and housing site-based work in varying weather conditions · Requires frequent travel to other counties, housing sites, micro shelter locations, and program offices · May involve irregular hours, evenings, or occasional weekends based on program, housing, and grant needs · Requires the ability to manage multiple priorities in fast-paced and evolving environments Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to walk, talk, and hear. The employee is also regularly required to sit and occasionally required to stand. Good manual dexterity is required for tasks involving the use of hands to operate a computer keyboard, handle objects, and perform detailed work. The employee is also required to reach with hands and arms. The employee must occasionally lift and/or move up to 50 pounds, including going up flights of stairs. Specific vision abilities required for this job include distance vision, color vision, depth perception, and peripheral vision.
