Overview We are seeking a compassionate and organized Care Coordinator to join our dedicated healthcare team. This role is vital in ensuring the delivery of personalized, high-quality care to individuals across various settings, including assisted living, nursing homes, and home care environments. The ideal candidate will possess strong communication skills, a thorough understanding of medical and caregiving practices, and a passion for improving the lives of those with disabilities, dementia, or other health challenges. Responsibilities * Coordinate and develop individualized care plans tailored to each resident’s or patient’s needs, including assistance with ADLs and medication management * Assist with activities such as meal preparation, light cleaning, and maintaining a safe environment * Monitor vital signs and assist with mobility using equipment like Hoyer lifts or transfer techniques * Support residents with dementia or Alzheimer’s through memory care strategies and behavior management * Document patient information accurately using EMR systems while ensuring compliance with HIPAA regulations * Assist with transportation needs by driving clients to appointments or activities as required * Collaborate with healthcare professionals, families, and support teams to ensure comprehensive case management * Provide direct support for residents’ emotional and physical well-being, including conflict resolution and behavioral support * Administer medications following prescribed protocols and document all care activities appropriately * Participate in ongoing training related to hospice care, autism spectrum support, or other specialized areas of caregiving Skills * Experience in assisted living, nursing home care, or home health environments preferred * Knowledge of medical terminology, patient care procedures, and EMR systems * Strong understanding of dementia care, Alzheimer’s care, memory care, and behavioral management techniques * Ability to assist with ADLs such as bathing, dressing, feeding, and mobility assistance including heavy lifting when necessary * Excellent communication skills for working with people with disabilities or developmental challenges * Valid driver’s license for transportation duties; safe driving record required * Basic first aid certification; CPR certification is a plus * Ability to handle conflict effectively and maintain professionalism in challenging situations * Compassionate caregiver with a focus on person-centered care and resident safety * Familiarity with HIPAA regulations to ensure confidentiality of patient information This role is essential in providing compassionate support that enhances the quality of life for individuals requiring specialized care. Job Overview We are seeking a Sports Physical Therapist to join our team. We are a performance-based sports physical therapy clinic dedicated to helping athletes return to sport stronger, faster, and more resilient than before injury. Our clinic bridges the gap between traditional physical therapy and true return-to-sport preparation. The ideal candidate will have OCS and/or SCS board certification, and will value evidence-based practice and outcome-driven rehab. Responsibilities Develop and implement individualized care plans tailored to patients’ specific sports injuries and physical needs. Conduct thorough assessments of patients’ musculoskeletal systems, utilizing knowledge of anatomy and physiology. Manage discharge planning for patients who have met benchmarks Provide therapeutic interventions such as therapeutic exercise, BFR, electrotherapy, manual therapy, and physiotherapy techniques to reduce pain and restore function. Monitor patient progress through EMR systems, adjusting treatment plans as necessary to ensure optimal outcomes. Educate patients on injury prevention, proper biomechanics, and home exercise programs to support ongoing recovery. Collaborate with physicians, coaches, and other healthcare professionals to coordinate comprehensive care strategies. Perform benchmark testing for Return to Sport assessments. Skills Strong knowledge of physical therapy principles with expertise in orthopedics, and sports medicine. Proficiency in clinical documentation within EMR systems. Excellent patient care skills with the ability to communicate effectively with diverse populations Ability to work collaboratively with team in fast-paced clinical settings.
