Resident Care Coordinator
Overview We are seeking a compassionate and organized Care Coordinator to join our dedicated healthcare team. This role is vital in ensuring the delivery of personalized, high-quality care to individuals across various settings, including assisted living, nursing homes, and home care environments. The ideal candidate will possess strong communication skills, a thorough understanding of medical and caregiving practices, and a passion for improving the lives of those with disabilities, dementia, or other health challenges. Responsibilities * Coordinate and develop individualized care plans tailored to each resident’s or patient’s needs, including assistance with ADLs and medication management * Assist with activities such as meal preparation, light cleaning, and maintaining a safe environment * Monitor vital signs and assist with mobility using equipment like Hoyer lifts or transfer techniques * Support residents with dementia or Alzheimer’s through memory care strategies and behavior management * Document patient information accurately using EMR systems while ensuring compliance with HIPAA regulations * Assist with transportation needs by driving clients to appointments or activities as required * Collaborate with healthcare professionals, families, and support teams to ensure comprehensive case management * Provide direct support for residents’ emotional and physical well-being, including conflict resolution and behavioral support * Administer medications following prescribed protocols and document all care activities appropriately * Participate in ongoing training related to hospice care, autism spectrum support, or other specialized areas of caregiving Skills * Experience in assisted living, nursing home care, or home health environments preferred * Knowledge of medical terminology, patient care procedures, and EMR systems * Strong understanding of dementia care, Alzheimer’s care, memory care, and behavioral management techniques * Ability to assist with ADLs such as bathing, dressing, feeding, and mobility assistance including heavy lifting when necessary * Excellent communication skills for working with people with disabilities or developmental challenges * Valid driver’s license for transportation duties; safe driving record required * Basic first aid certification; CPR certification is a plus * Ability to handle conflict effectively and maintain professionalism in challenging situations * Compassionate caregiver with a focus on person-centered care and resident safety * Familiarity with HIPAA regulations to ensure confidentiality of patient information This role is essential in providing compassionate support that enhances the quality of life for individuals requiring specialized care.
