Corewell Health is seeking a BE/BC Family Medicine Physician to join our team at our Allendale, MI primary care clinic near Grand Valley State University campus. In this role, you will work alongside other Physicians and APP’s caring for the patients of Allendale, surrounding communities, and college campus. Work a four-day work week with EPIC EMR and 3 Physician and 3 Advanced Practice Provider colleagues. Team-based care is supplemented by a behavioral health specialist, care manager, medical assistants and experienced operations manager. May also analyze external cardiacs and assist in the implantable device management. Provides patient education related to heart healthy lifestyle choices. Plans, develops and conducts graded exercise testing. Monitors patient’s activity, physiological response, and adherence to the exercise sessions. Provides specific exercise prescription to individuals based on age, culture and orthopedic considerations. May also analyze external monitor findings utilizing appropriate technology and processes report according to protocol; and assist in management of patients with implantable devices, including but not limited to triage, entering and tracking data, reviewing remotes, performing in-clinic interrogations, and scheduling patients as needed. Provides patient education talking points and/or individual and group counseling and teaching regarding principles of a healthy lifestyle. May create and update educational materials. Assists with new employee orientation within department. Communicates and collaborates with other members of the health care team in order to ensure continuity of care and coordination of services. Participates in research projects and on committees/task forces as member, consultant and/or chair. May flex to the medical program or surgical program as needed.
