Licensed Practical Nurse – Memory Care

**LPN – Memory Support (Night Shift)** Provide compassionate, person\-centered nursing care to residents with memory\-related conditions. Administer medications, monitor and document health status, and support daily living activities while promoting dignity, safety, and comfort. **Key Responsibilities** * Deliver individualized care aligned with resident needs and care plans * Administer prescribed medications and follow medication safety practices * Monitor residents’ physical and behavioral status; report changes to the care team * Assist with activities of daily living and support independence where possible * Maintain a safe environment and respond appropriately to health concerns * Collaborate with an interdisciplinary team to coordinate care * Communicate effectively with families and support empathetic, respectful interactions **Qualifications \& Skills** * Licensed Practical Nurse (LPN) with a strong commitment to compassionate care * Ability to provide supportive care for individuals with memory\-related conditions * Strong medication administration and health monitoring skills * Excellent communication and teamwork skills * Dedication to resident safety, comfort, and dignity
