DescriptionA Brief Overview The Advanced Care Paramedic is responsible for providing advanced lifesaving interventions to patients and serves as the principal paramedic for specialty care and ALS transports. This position provides care for both adult and pediatric patients during specialty care transports, on-site care interventions for acute hospital, home visits, Industrial Health locations, correctional health service locations, and event medicine. The Advanced Care Paramedic will function at the highest levels within their scope of practice and make advanced clinical choices. This position will invest substantial time in obtaining and maintaining advanced certifications (i.e., ALS, PALS, ITLS, GEMS). Additionally, the Advanced Care Paramedic may serve as a Clinical Care Technician, consistent with the roles and responsibilities as defined by the position, to provide support for in-patient clinical services such as cardiac catheter lab, ICU, Ed, and other clinical areas where the knowledge and skill of the Advanced Care Paramedic contributes to the patient care plan. What You Will Do * Practices critical clinical decision-making and delivers care to patients at the top level of the Paramedic’s scope of practice in a team-based care delivery model with the mission to provide initial and/or continuing care to stabilize patients in any environment (residence, clinic, ED, hospital) and as required, provide or facilitate transport via ambulance. * Provides care and transport to adult and pediatric patients at all acuity levels within the paramedic scope of practice including but not limited to STEMI, stroke, trauma, emerging or re-emerging infectious diseases, and high-risk OB patients. * Completes appropriate charting and clinical documentation in a team-based approach. * Displays a commitment to the EMS-I Team and UH System's mission, vision, and values. * Advocates unwavering for patient and crew safety. Additional Responsibilities * Supports public relations events to represent the team and University Hospitals as required. * May be assigned to support inpatient clinical services. * Staffs UHHS Event Medicine as requested. * Performs other duties as assigned. * Complies with all policies and standards. * For specific duties and responsibilities, refer to documentation provided by the department during orientation. * Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. QualificationsEducation * High School Equivalent / GED (Required) * Other Accredited Program: Graduated from a state accredited paramedic program (Required) Work Experience * 3+ years experience in a busy 911 ALS EMS system (Required) or * 1+ years experience as a Paramedic (Required) Knowledge, Skills, \& Abilities * Medical Assistant skill set upon completion of Orientation (Required proficiency) * Pharmacy Technician skill set upon completion of Orientation (Required proficiency) * Excellent interpersonal and communication skills both written and oral (Required proficiency) * Strong organizational and multi-tasking skills (Required proficiency) * Demonstrates a professional demeanor when interacting with patients and customers (Required proficiency) * Strong computer skills including all Microsoft platforms (Required proficiency) * Excellent ability to perform triage in order to determine appropriate facility, bed type, service, etc. (Required proficiency) * Must excel at working in an interdisciplinary environment inclusive of physicians, advanced practicing providers, nursing, respiratory therapy, and other members of the clinical care team. (Required proficiency) * Working knowledge of and experience with advanced equipment consistent with the standard competencies of the Advanced Care Paramedic scope of practice inclusive of vent management, use of high-flow vents, and advanced airway management. Job Summary The Community Based Care Coordinator, Duals Integrated Care is responsible for managing and coordinating care for dual-eligible beneficiaries, those who qualify for both Medicare and Medicaid. This position focuses on integrating health services and community resources to improve health outcomes and enhance the quality of life for individuals with complex health needs, including those who are eligible for waiver services. Essential Functions * Engage with the member in a variety of community-based settings to establish an effective, care coordination relationship, while considering the cultural and linguistic needs of each member. * Function as a liaison between healthcare providers, community resources, and dual-eligible beneficiaries to ensure seamless communication and care transitions. * Conduct comprehensive assessments to identify the physical, mental, and socials needs of dual-eligible individuals. * Develop and implement individualized care plans based on unique needs of each member, considering their medical, social, and behavioral health requirements. * Lead and collaborate with interdisciplinary care team (ICT) to create holistic care plans that address medical and non-medical needs. * Assist members in accessing community resources, including housing, transportation, food assistance, and social services. * Educate members about their benefits and available services under both Medicare and Medicaid. * Provide education to members and their families about managing chronic conditions, medication adherence, and preventive care. * Promote health lifestyle choices and self-management strategies. * Regularly monitor member’s health status and care plan adherence, adjusting, as necessary. * Follow up with members after hospitalizations or significant health events to ensure continuity of care and prevent readmissions. * Work closely with primary care physicians, specialists, and other healthcare providers to coordinate care and share relevant information. * Coordinate with community-based organizations, other stakeholders/entities, state agencies, and other service providers to ensure coordination and avoid duplication of services. * Participate in care team meetings to discuss member progress and address barriers to care. * Maintain accurate and up-to-date records of members interactions, care plans, and outcomes. * Collect and analyze data to evaluate the effectiveness of care coordination efforts and identify areas of improvement. * Advocate for the needs and preferences of dual-eligible beneficiaries within the healthcare system. * Empower members to take an active role in their healthcare decisions. * Evaluate member satisfaction through open communication and monitoring of concerns or issues. * Regular travel to conduct member, provider and community-based visits as needed and per the regulatory requirements of the program. * Report abuse, neglect, or exploitation of older adults as a mandated reporter as required by State law. * On-call responsibilities as assigned. * Adherence to NCQA and CMSA standards. * Perform any other job duties as requested. Education And Experience * Nursing degree from an accredited nursing program or bachelor’s degree in a health care field or equivalent years of relevant work experience is required. * Previous experience in nursing or social work or counseling or health care profession (i.e. discharge planning, case management, care coordination, and/or home/community health management experience) is required. * Prior experience in care coordination, case management, or working with dual-eligible populations is preferred * Medicaid and/or Medicare managed care experience is preferred Competencies, Knowledge And Skills * Intermediate proficiency level with Microsoft Office, including Outlook, Word and Excel. * Understanding of Medicare and Medicaid programs, as well community resources and services available to dual-eligible beneficiaries. * Strong interpersonal and communication skills to effectively engage with members, families, and healthcare providers * Ability to manage multiple cases and priorities while maintaining attention to detail. * Adhere to code of ethics that aligns with professional practice. * Awareness of and sensitivity to the diverse backgrounds and needs of the populations served * Decision making and problem-solving skills. Licensure And Certification * Current unrestricted clinical license in state of practice as a Registered Nurse, Social Worker or Clinical Counselor is required. Licensure may be required in multiple states as applicable based on State requirement of the work assigned. * Case Management Certification is highly preferred * Must have valid driver’s license, vehicle and verifiable insurance. Employment in this position is conditional pending successful clearance of a driver’s license record check and verified insurance. If the driver’s license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in the position will be terminated. * To help protect our employees, members, and the communities we serve from acquiring communicable diseases, Influenza vaccination is a requirement of this position. Compensation Range $62,700.00 - $100,400.00 CareSource takes into consideration a combination of a candidate’s education, training, and experience as well as the position’s scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
