Hospice Clinical Liaison / Admissions Specialist (RN / LPN / LVN / Hospice Clinicians Encouraged to Apply) Location: Within 60 mile radius of Clackamas, OR 97015 Salary: $80,000-$110,000/per year + bonuses Schedule: Monday-Friday | Full-Time | Flexible Schedule Use Your Clinical Expertise—Without the Bedside Burnout Agape Hospice NW is seeking a Hospice Clinical Liaison / Admissions Specialist with strong hospice or post-acute care knowledge to help educate families, build referral relationships, and guide patients into compassionate end-of-life care. This role is ideal for hospice clinicians or SNF nurses who want to transition from bedside care into the business and operations side of hospice—while still making a meaningful patient impact.
