Responsibilities * Assist residents or clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. * Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; observe for changes in condition and report findings promptly. * Support medication administration following HIPAA guidelines to ensure privacy and safety. * Prepare nutritious meals tailored to dietary needs while maintaining cleanliness and sanitation standards in the kitchen area. * Implement care plans designed by healthcare professionals to meet individual needs effectively. * Provide specialized dementia and Alzheimer's care, including memory care techniques and patient observation to ensure safety and engagement. * Maintain a clean and safe environment by performing cleaning duties within resident spaces and common areas. * Document care activities accurately in compliance with healthcare regulations and facility policies. * Support patients during community outings or home visits, ensuring their safety during transportation with equipment like Hoyer lifts or wheelchairs. * Collaborate with interdisciplinary teams including social workers, nurses, and therapists to deliver holistic care tailored to each individual’s needs. Experience * Prior experience working in assisted living, nursing homes, group homes or home \& community care settings is highly desirable. * Proven ability to work with individuals with developmental disabilities, autism, dementia or memory-related conditions such as Alzheimer’s disease. * Hands-on experience with patient observation, vital signs monitoring, medication administration, and care planning. * Knowledge of HIPAA regulations to protect patient privacy at all times. * Familiarity with caregiving tools like Hoyer lifts or resident transfer techniques; ability to perform heavy lifting safely when required. * Experience in providing hospice or end-of-life care is a plus. * Strong interpersonal skills to build trusting relationships with residents and their families while demonstrating compassion and professionalism at all times.
