Patient Care Coordinator
Job Summary The Patient Care Coordinator acts as the primary liaison between our clients and our healthcare team. You will be responsible for managing the patient journey from the moment they sign up—onboarding them, scheduling the right nursing or caregiver staff, and monitoring the ongoing quality of care. Your goal is to ensure a seamless, high-quality experience for the patient while optimizing staff efficiency. Key Responsibilities * Case Onboarding: Review patient medical records and assessment notes to understand the specific level of care required (e.g., ICU-at-home, palliative care, or basic assistance). * Staff Rostering: Match and assign the most suitable nurses, caregivers, or therapists to cases based on skill set, location, and patient preference. * Coordination \& Logistics: Manage daily schedules and handle emergency replacements or shift changes if a staff member is unavailable. * Quality Assurance: Conduct regular "check-in" calls with families to gather feedback and ensure the care plan is being followed correctly. * Clinical Liaison: Relay updates from the field staff (nurses/caregivers) to the patient’s family and, if necessary, their primary physician. * Billing \& Documentation: Ensure all care logs, attendance records, and billing details are accurate and updated in the system. * Escalation Management: Serve as the first point of contact for any grievances or service issues, resolving them promptly to maintain client trust. Required Skills \& Qualifications * Experience: 1-2 years in healthcare coordination, hospital administration, or homecare operations. * Medical Literacy: Basic understanding of medical terminology and homecare procedures is highly preferred. * Multitasking: Ability to manage multiple "moving parts" (staff schedules, patient emergencies, and family requests) simultaneously. * Conflict Resolution: Strong diplomatic skills to handle sensitive family dynamics or staff grievances. * Tools: Proficiency in scheduling software, CRM tools, and WhatsApp/Communication suites for team coordination. * Availability: Must be comfortable being "on-call" occasionally for weekend or after-hour emergencies. Performance Indicators (KPIs) * Staff Utilization: Keeping caregivers/nurses consistently assigned to active cases. * Retention Rate: High levels of patient satisfaction and low service cancellations. * Turnaround Time: How quickly a caregiver is deployed once a family signs a contract.
