Physical Therapist – Ascend Medical About Us Ascend Medical is a growing outpatient practice specializing in interventional pain management, physical therapy, and sports performance. Our integrated care model means physical therapists and physicians work as partners—each bringing their expertise to help patients get back to the activities they love, whether that's competitive athletics, everyday movement, or returning to work after an injury. The Role We're looking for a Physical Therapist who wants to do meaningful work. You'll treat a diverse orthopedic and sports medicine caseload, practice with full clinical autonomy, and have colleagues across disciplines when you want to collaborate on complex cases. What You'll Do Evaluate and treat patients with orthopedic injuries, post-surgical conditions, chronic pain, and sports-related injuries Develop individualized treatment plans and progress patients toward functional goals Practice independently while having access to an in-house team of pain management specialists for multidisciplinary cases Document evaluations, daily notes, and progress reports in our EMR system Educate patients on exercise programs, injury prevention, and self-management strategies What We're Looking For New York State Physical Therapy license (required) Doctor of Physical Therapy degree from an accredited program Outpatient orthopedic experience preferred, but strong new grads with ortho clinical rotations are welcome to apply Manual therapy skills and an interest in working with an active patient population Reliable, team-oriented, and genuinely interested in patient outcomes Why Ascend Medical Full clinical autonomy—you drive your patients' PT care Integrated setting with access to pain management specialists when it benefits your patients Manageable caseload that allows quality treatment time Supportive administrative team (you focus on patients, we handle the rest) Opportunities for professional development and growth as we expand Pay: $90,000.00 - $110,000.00 per year Benefits: 401(k) 401(k) matching Health insurance * Paid time off Work Location: In person
