This is a Full Time Position Compensation Range: $18.86 TO $27.60 - Certified/Registered Medical Assistant $22.26 to $33.38 - Licensed Practical Nurse $30.75 to $46.13 - Registered Nurse Benefits: Medical, Dental, Life, Retirement, Paid Time Off Position Classification: Non-Exempt ESSENTIAL FUNCTIONS AND BASIC DUTIES: Supervisory-Specific Performance Expectations, Duties, and Responsibilities: N/A Position-Specific Performance Expectations, Duties, and Responsibilities: Annual Wellness Visits (AWV): Completes pre-visit planning and identifies patient needs for age-appropriate screenings and immunizations, in collaboration with MRH’s family practice providers. Provides education to patients about their conditions within the scope of practice. Updates health records utilizing the tools in the electronic health record. Documents patient visits in electronic health records in a timely manner. Value-Based Care (VBC): Supports data-driven VBC initiatives for Medicare, Medicare Advantage, Medicaid, and commercial insurance plan beneficiaries. Includes direct patient outreach for medication adherence and collaboration with Primary Care Physicians (PCP) and clinic staff to monitor and follow up with high-risk and impactable patients identified through claims data as needing wellness visits, follow-up appointments, lab work, and other care. Lifeline: Primary point of contact for enrollment, equipment set-up, and technical support of personal emergency response system (PERS) devices. Reviews monthly invoices to ensure correct billing. Assists MRH family practice providers with population health initiatives for specific diagnoses, i.e., diabetes, CHF, COPD, hypertension, etc. Team-Based care (TBC). Provides relevant health care information to Family Practice Providers to help assess if patients would benefit from Chronic Care Management (CCM); enter Referrals on behalf of Providers for CCM, Patient Navigation, and/or social Care Coordination for ongoing support, as applicable. Has an existing understanding of Medicare Annual Wellness Visits (MAWV) and Chronic Care Management (CCM) services, OR has an interest and ability to learn about MAWV and CCM; ability to explain the purpose and value of MAWV and CCM to patients. Performs other duties as assigned. Estimated time allocation: AWV 80%; VBC 10%; Lifeline 10%. General Requirements Maintains accurate documentation of patient encounters and ensures timely reporting according to department and organizational standards and requirements. Attends recurring staff meetings and trainings as assigned. Participates in the development of processes to achieve improved health outcomes and programmatic efficiencies. Meets patient onsite at the clinic/hospital, public spaces, homes, or other appropriate locations to assess needs and provide support. Communicated professionally and effectively with patients, community members, professional partners, grantors, and in other professional settings. Possesses critical thinking and problem-solving skills, uses data to make informed decisions, and is both self-directed as well as a collaborative team member. Occasional weekend work, and other duties as assigned. Strives to uphold regulatory requirements to ensure continual compliance with departmental, hospital, state, and federal regulations and policies. QUALIFICATIONS: Minimum Requirements: Must be at least 16 years of age (21 for driving positions). Must be able to legally work in the United States. Must be able to pass a background check. Must be able to pass a drug screen and breath alcohol test (if applicable). Must complete employee health meeting. Required Education/Licensure/Certification: Must have one of the following (see grading on page 1): + Current Unencumbered Colorado RN license or Compact license. + Current Unencumbered Colorado LPN license or Compact license. + Registered or Certified Medical Assistant Current BLS certification (or must be obtained within 90 days). Experience: One (1) year nursing experience in primary care, community nursing, critical care, nursing home, or home health, preferred. Skills/Abilities: Ability to work independently and exercise clinical judgment in interactions with providers, patients, and their families. Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple tasks and role components. Aware of the scope of practice boundaries, comfortable seeking direction and assistance from appropriate resources. Work within an electronic health record to chart and review patient data and run reports. Communicate effectively, both in writing and verbally. Establish and maintain effective working relationships with employees at all levels throughout the organization. Interpret, adapt, and apply guidelines and protocols. Identify and resolve problems in a proactive, collaborative manner. Commitment and leadership in regard to advancing diversity and inclusion. The Wellness Coordinator is responsible for planning, organizing, and leading wellness, health and fitness programs, in a safe manner while developing and monitoring budgets for these programs. The Wellness Coordinator may be trained to support the efficient administrative management of rehabilitation services in their assigned accounts. RESPONSIBILITIES/ACCOUNTABILITIES: Designs and implements personalized 1:1 fitness training programs tailored to help older adults achieve their individual fitness goals, taking into account their physical capabilities and health conditions. Designs and implements health and wellness programs that reflect the Powerback Rehabilitation concepts and practices in response to participants’ needs. Ensures that cardiovascular and strength training equipment is appropriate to the population served and maintained in accordance with manufacturer specifications; coordinates repair (as needed) and regular preventive maintenance of all exercise equipment. Recommends acquisition of exercise equipment to support new programs and activities. Ensures the development of programming supports Powerback Rehabilitation and the organization’s philosophy of serving the needs of the residents. Establishes procedures to evaluate programs and activities from revenue, expense, and participant satisfaction perspectives. Develops, promulgates, and implements a safety plan for wellness area activities, including procedures to follow in the event of an emergency. Ensures that cost-effective procedures are in place to provide the highest level of programming and safety for employees, residents, guests and visitors. Completes the patient registration process, patient scheduling, and daily staffing with direction from the DOR. Develops and maintains key relationships with the facility management team and staff as the on site representative of Powerback Rehabilitation. Provides “Client related” and “Non-Client related” services to patients as directed by Therapists and Assistants within the parameters of: State practice laws; Local, state, and federal laws and regulations; Reimbursement rules and payor policies; Professional standards of practice and codes of ethics. In this role, the Wellness Coordinator will: Manage the fitness center operation including facilities, personnel, programs and budget. Lead group exercise classes, continuously monitoring and adjusting types of classes and times to meet the interest and needs of residents. Programming will include purpose based and functional fitness classes as well as traditional fitness classes appropriate to senior living communities. Actively encourage resident participation in the fitness center and classes. Promote and facilitate the use of the Technogym, other fitness machines in the fitness center. Facilitates senior fitness tests with residents; enter information into LifeConnect or Technogym technology. Develop individual comprehensive fitness program that addresses individual resident’s needs and abilities in all levels of care. Provide personal and fitness instruction for residents, which include equipment orientation, a personal fitness assessment and monitoring ongoing-progress of residents. Work with therapy director, residential nurse, and healthcare administrator to present preventive health programs, conduct balance assessments, and ensure provision of rehabilitative services as medically indicated. The ideal candidate will have: Bachelor’s degree, preferably in a related field such as physical therapy, exercise physiology, gerontology. Current American College of Sports Medicine (ACSM) Health Fitness Specialist or Group Health Instructor required at time of or within 3 months of hire. Specialty certificate highly desired such as personal trainer, Tai Chai, or Arthritis Foundation aquatic certification. A.C.S.M. certification must be kept current. CPR and First Aid Certification are required and must be kept current. Experience working with senior adults or equivalent knowledge and experience in the long-term care and health care industry required. Two years’ experience in direct fitness programming required; experience in a senior adult setting highly desirable.
