HOME PREFERRED SENIOR CARE Home Health PRN Occupational Therapist - Odessa, TX JOB DESCRITION I.QUALIFICATIONS A.Graduate of an American Occupational Therapy Association approved physical therapy program. ------------------------------------------------------------------------------------------------- B.Current license to practice physical therapy in Texas. ------------------------------------------------------------ C.Current drivers license; with current automobile insurance. ----------------------------------------------------------------- II.JOB FUNCTIONS ==================== A.Conducts patients’ assessments and assists the physician in evaluating the level of function by applying diagnostic and prognostic functional ability tests. ------------------------------------------------------------------------------------------------------------------------------------------------------------------ B.Assist in the development of the Plan of Care. ---------------------------------------------------- C.Directs and aids patients in active and passive exercise, muscle re-education, as well as gait, functional, ADL, transfer, and prosthetic training. ---------------------------------------------------------------------------------------------------------------------------------------------------------- D.Treats patients to relieve pain; develop and restore function; and maintain maximum functioning using means such as exercise, massage, heat, water, light, and electricity. --------------------------------------------------------------------------------------------------------------------------------------------------------------------------------- E.Makes use of ultraviolet and infrared lamps, diathermy, ultrasound, whirlpool, contract baths, and applies moist packs. ----------------------------------------------------------------------------------------------------------------------------- F.Observes, records, and reports to the physician the patient’s response to treatment and changes in the patient’s condition. --------------------------------------------------------------------------------------------------------------------------------- G.Instructs patients, significant others, and staff in the care and use of wheelchairs, prosthetic, and orthopedic devices; and in areas of physical therapy in which they can participate and assist the patient. ---------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------- H.Visits patient according to Plan of Treatment; completes a progress note for each visit; and submits progress notes to the agency on an at least weekly basis. -------------------------------------------------------------------------------------------------------------------------------------------------------------------- I.Participates in staff conferences and committees as necessary. -------------------------------------------------------------------- J.Attends all education and in services required ---------------------------------------------------- K.Prepares a written discharge summary and a written summary report on patients condition at least every 60 days. --------------------------------------------------------------------------------------------------------------------- III.RELATIONSHIPS: ====================== A.Supervised by Director of Nurses and/or Alternate Director of Nurses -------------------------------------------------------------------------- B.Supervises: ----------------- ### 1.Other Occupational and occupational therapy assistants in their employment
