Care Coordinator

The role is fully paid and suitable for individuals who are organised, empathetic, and committed to delivering excellent service. **Responsibilities** * Coordinate and oversee individual care plans in collaboration with clients, families, and healthcare professionals * Schedule appointments and manage care delivery timelines effectively * Maintain accurate and up\-to\-date client records using office management systems * Liaise with service providers to ensure seamless delivery of care services * Provide support and guidance to clients, addressing their needs and concerns promptly * Organise documentation related to client care, including reports and progress notes * Assist with administrative duties such as filing, data entry, and correspondence * Ensure compliance with organisational policies and relevant regulations **Skills** * Proven experience in an office environment, demonstrating strong organisational skills * Excellent communication skills, both written and verbal * Compassionate approach with the ability to build rapport with clients from diverse backgrounds * Strong organisational abilities with attention to detail * Ability to prioritise tasks effectively in a fast\-paced environment * Proficient in using office software such as Microsoft Office Suite or similar programmes * Ability to work independently as well as part of a team * Empathy and patience when dealing with sensitive situations This role offers a rewarding opportunity for individuals passionate about providing high\-quality care coordination within a professional setting.
