Hospice Social Worker

**Overview** St. Martin High Desert Hospice is shaping itself into a world\-class facility and patient experience. We are seeking a compassionate and professional **Hospice Social Worker** (BSW or MSW) who demonstrates strong hospice knowledge, polished representation, and excellence in documentation and communication. This role is essential in supporting patients and families with psychosocial needs, advocacy, and access to resources that improve quality of life. An MSW fulfills the Medicare regulatory requirement for hospice social work. A BSW may be considered for this position under the supervision of an MSW, in accordance with federal and state guidelines. **Key Responsibilities** * Provide psychosocial support, counseling (MSW), and ongoing guidance to patients and families aligned with hospice philosophy. * Conduct assessments (MSW required for primary clinical assessments; BSW may assist under supervision). * Maintain **timely, accurate, and professional documentation** in the EMR, meeting hospice compliance standards. * Represent SMHD with professionalism, dignity, and empathy in every patient and family interaction. * Collaborate with the interdisciplinary group (IDG) to ensure holistic care planning and delivery. * Connect patients and families to appropriate **community resources**, including: **SNAP (Supplemental Nutrition Assistance Program), IHSS (In\-Home Supportive Services), ect.** * Housing, transportation, financial, and caregiver support programs * Provide crisis intervention, conflict resolution, and family support in difficult care situations. * Manage caseload with strong **time management**, prioritizing visits and documentation efficiently. **Qualifications** **Education:** * MSW (Master of Social Work) from an accredited program **preferred**. * BSW (Bachelor of Social Work) from an accredited program **considered** with supervision from an MSW. **Licensure:** * California LCSW preferred (not required). **Experience:** * Minimum 1 year in hospice, palliative care, or healthcare social work strongly preferred. **Skills:** * Knowledge of hospice philosophy and Medicare/Medicaid hospice regulations. * Strong communication, documentation, and professional representation. * Ability to connect patients/families to local/state resources. * Compassion, empathy, and the ability to handle sensitive end\-of\-life situations effectively. **Compensation \& Benefits** * Competitive per\-visit or salary pay structure * **Dental Insurance** * **Vision Insurance** * **Life Insurance** * Supportive interdisciplinary team environment * Opportunity to be part of a growing organization committed to **world\-class hospice care** Job Type: Full\-time Pay: $80,000\.00 \- $95,000\.00 per year Work Location: In person
