Adult Care Coordinator

The Care Coordinator will be responsible for monitoring client progress, ensuring that all services are provided in a timely manner, and working with clients to ensure their needs are met. Responsibilities: * Develop and implement individualized care plans for clients. * Educate clients on health and wellness topics. * Write the Plan of Care for each member. * Identify and link members with community resources. * Facilitate referrals and respond to social service needs. * Collaborate with other team members including doctors, nurses, other care coordinators, and clinicians. * Help members implement the Plan of Care. * Monitor and assist them in problem solving potential issues related to the health care system, financial or social barriers. * This roll will also support and encourage members in actively participating in reaching the goals they set together! If you have 1 year of experience in mental health work (MHRT\-1\) this is the perfect opportunity of growth! * Excellent communication skills for effective interaction with clients and healthcare professionals. * Ability to work collaboratively within a team environment. * Organizational skills to manage multiple client cases efficiently. * MHRT/C, MHRT/Cp OR at least 1 year working as a Peer Support, Recovery Coach or in the Daily Living Skills field.
