Discharge Case Manager

Chandler Post Acute is in need of an energetic, well organized Case Manager to join our Leadership Team! As our Case Manager, you will facilitate a seamless transfer from one level of care to the next by providing coordination and arranging post\-discharge services. These services include arranging Home Healthcare, Extended care\-services, Medical equipment, and Transportation.
