Job Duties* Participate in a multidisciplinary team to develop, execute, monitor, and adjust comprehensive and coordinated care plans that include shared goals with measurable outcomes. * Serve as primary point of contact and coordinator of care for assigned patients. * Assist patients with Social Service needs as necessary. * Participate in Peer to Peer or Care Management reviews with patient relations liaison. * Ensure the Unitized Transdisciplinary Care program and units are scheduled and prepped for presentation. * Present patients to provider teams during care reviews. * Complete patient Care reviews as needed. * Provide nursing services including assist providers with examinations and procedures; administer medications and injections to patients; check patient's blood glucose, process referrals to external providers and community health resources; and relay provider instructions to patients. * Document medical and behavioral encounters in the health record system. * Maintain and stock the clinic STAT Kits, AED and O2 tanks on a regular basis. * Complete follow-ups on behalf of patients and providers including outreach phone calls. * Engage patients and provide care in individual and group settings that focus on achieving care plan goals and education. * Care for a medically complex and emotionally challenging patient population that requires compassion, empathy, and the ability to deescalate and appropriately react to unacceptable behavior in individual and group settings. * Encourage patients to take control and responsibility of their health through self-management and use of community resources. * Participate to be On-Call and be available to take patient calls after hours via phone. Qualifications* Current Vocational Nurse license in the state of California. * General computer skills including typing, Microsoft Word, Microsoft Excel, Microsoft Outlook, and medical record data review and entry. * Bilingual (English/Spanish) preferred. Onoria Healthcare Provider Inc. is a well known Home Health Care agency in the San Bernardino County that currently provides home care to primarily the elderly patient community and is currently seeking an LVN Care Coordinator to work full time in an office setting in the city of Montclair with occasional home visiting as needed. The Care Coordinator must be a professional Licensed nurse who assists in the coordination of the care plan and coordinates the services provided to patients. Care coordinator will also assist in the scheduling of patient visits. Candidate must have strong Interpersonal skills and will work collaboratively with other members of the interdisciplinary team to ensure the safe and efficient transition between Hospital or MD office care and care in the home setting. Job Responsibilities include: 1. Obtains and completes timely reports from RN and other disciplines and inputs into EMR system. 2. Communicates/relays reports and new MD orders over to assigned field nurses and any other involved disciplines pertinent the a particular patient. 3. Coordinates with scheduler to assure timely staffing of all disciplines needed per plan of care and assists with staffing when deemed necessary. 4. Ensures accuracy of admissions and discharges into the EMR system. 5. Follows-up routinely with wound care centers and MD offices to obtain any new orders post appointments and relays them to assigned nurses on the case to be carried out appropriately and timely. 6. Orders Medical equipment, Medication refills, IVs, Labs, X-rays or other services required and ordered by the physician. 7. Communicates will all members of the multidisciplinary team to improve the quality of patient care. 8. Participates in case conferences and makes regular entries in pertinent logs as needed. 9. Maintains knowledge and adheres to all company policies and procedures and regulatory standards. 10. All other tasks and duties assigned by management and deemed necessary and appropriate. The ideal candidate Must be flexible, reliable, dependable, self-starter with excellent Interpersonal skills and ability to work in a fast paced multitasking environment.
