We are seeking a dedicated MDS/Care Plan Coordinator to ensure that resident assessments and care plans are accurate, up-to-date, and aligned with the care provided. This position plays a key role in communicating resident conditions to families, coordinating care plans, and ensuring compliance with regulatory requirements. MDS/Care Plan Coordinator Requirements: * Current State of Illinois LPN license * AANAC certification preferred * MDS experience required * Strong organizational, planning, and managerial skills * Working knowledge of nursing services, nursing administration, rehabilitation, general and geriatric nursing, MDS documentation, and EMR systems * Ability to initiate, complete, and update care plans efficiently * Experience conducting staff training on care plans, documentation, and EMR use * Ability to monitor resident EMR records for consistency and accuracy * Strong interpersonal skills for effective communication with residents, families, and staff * Knowledge of JCAHO, OBRA, IDPH, and HFS documentation standards As an MDS/Care Plan Coordinator, you will: * Oversee the completion of MDS assessments upon admission, readmission, quarterly, annually, and during significant changes or PPS/Insurance Reimbursement periods * Develop and maintain a monthly MDS assessment and Care Plan Conference schedule * Ensure timely completion of MDS sections by the appropriate department and validate accuracy * Collaborate with departments to identify and resolve MDS-related issues, providing re-education as needed * Review diagnosis coding and sequencing with physicians quarterly, updating ICD-9/ICD-10 coding as necessary * Lead weekly MDS Pre-Planning and Medicare meetings to ensure compliance and accuracy * Assist and educate staff in EMR charting, documentation, and achieving Care Plan goals * Communicate resident care plans and progress to staff, residents, and families * Conduct quarterly in-service training for nursing staff on care plans and documentation * Audit monthly EMR charting to ensure proper documentation of care and follow up with staff when needed * Report and follow up on documentation discrepancies to ensure regulatory compliance * Work closely with the Director of Nursing and interdisciplinary team to maintain consistency in resident care * Coordinate resident Care Plans with therapy departments (physical, occupational, respiratory, and speech therapy) * Ensure proper nutritional assessments are completed and documented, collaborating with dietary consultants * Maintain resident confidentiality and adhere to fire, disaster, safety, infection control, and evacuation policies A Workplace That Cares About You! Our proprietary Flexologist™ Training Program (FTP) ensures that Flexologists™ receive world class training and their clients receive a world-class stretching session. StretchLab Willowbrook goes the extra mile. We spend additional time and effort preparing you, in studio, even before FTP, so that you can absorb and maximize training. After FTP, we will spend more time with you so that you are 100% prepared to give that quality experience to our clients. This position is for 20-30 hours a week. It pays a base rate based on experience in addition to bonuses for stretching each client. Total compensation will vary due to how many hours you are working with a client (utilization rate). Total hourly compensation is typically between $18-$24. Total compensation will trend higher as you build a devoted clientele. StretchLab employees get discounted pricing for all retail merchandise. Essential Duties \& Responsibilities: Deliver one-on-one assisted stretch sessions as well as group stretch classes for up to 6 clients. Encourage and motivate clients throughout stretch sessions. Build StretchLab membership and retain current clientele. Ensure safety of clients in regards to proper stretch techniques and enforce StretchLab polices and safety rules. Attend staff meetings and required educational presentations. Handle member concerns when applicable. Assist Sales Associates and General Manager with studio tour and sales as needed. Clean and maintain the studio in order to ensure it is available for client use at any given time. Deliver one-on-one assisted demo stretch sessions at offsite promotional events. Be available to work a minimum of 3 days a week (could include mornings, midday shifts, or evenings).
