Overview We are seeking a dedicated and compassionate Care Co-ordinator to join our team. This role involves organising and managing care plans to ensure individuals receive the appropriate support tailored to their needs. The ideal candidate will possess excellent organisational skills, a caring attitude, and experience in office administration. Duties * Coordinate and schedule care appointments, ensuring timely delivery of service * Maintain accurate and up-to-date records of all interactions and care activities * Organise resources and allocate staff efficiently to meet client requirements * Provide administrative support including data entry, filing, and correspondence management * Monitor service delivery to ensure compliance with organisational standards and regulations * Respond promptly to enquiries from clients or stakeholders, providing clear information and assistance * Participate in the on-call rota and support staff outside office hours * Attend face-to-face client assessments and reassessments when required * Attend review meetings with clients, families, social workers, and other professionals when requested Skills * Previous office experience is desirable, demonstrating strong organisational and administrative capabilities * Excellent communication skills, both written and verbal, with a compassionate approach * Ability to prioritise tasks effectively in a fast-paced environment * Proficient in using office software such as Microsoft Office Suite or similar programmes * Strong attention to detail and organisational skills to manage multiple cases simultaneously * Empathy and professionalism when dealing with vulnerable individuals and their families This role is vital in ensuring seamless coordination of care services, contributing positively to the wellbeing of those supported.
