Community Mental Health Practitioner
The post holder is responsible for the assessment of care needs and contributing to the formulation of the child and family's needs and the development of a relevant plan based upon the child and family's negotiated goals and relevant evidence-based interventions. The post holder, in conjunction with the multi-disciplinary team, will provide / mental health evidenced based interventions to support the child and family to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the child and family throughout their care They will also be expected to work flexibly within the Team to support a crisis, brief intervention and intensive support function. Dependent on skill and service need they will need to be available to external professionals and carers to offer advice, signposting and guidance (especially in relation to urgency and risk) associated with children and young people's mental health and emotional wellbeing. Postholder will provide assessments and interventions for CYP as part of the MDT for those experiencing severe to moderate mental health conditions. Person Specification Qualifications and Training Essential • First level (Degree or higher diploma) professional Nursing / AHP / Social Work / Therapy qualification in mental health, Social Work, Nursing AHP or equivalent with demonstrable knowledge and continuous post qualified experience/professional development • Conforms to Professional governing body requirements Desirable • Recognised appropriate qualification in mentoring/assessing/supporting students in placement • Professional specific additional qualification i.e non-medical prescriber (NMP), Sensory Integration , Approved Mental Health Practitioner (AMHP) • Psychological intervention/Professional Qualification including, Motivational Interviewing techniques CBT, CBTp, Family Interventions, CAT, DBT, Solution focussed interventions, Positive Behaviour Support Planning, Experience Essential • Evidence of extensive post registration experience with working with those who may experience complex mental health needs in the community setting Experience of working with people who have difficulty in engaging with services and have complex mental health needs • Experience of assessing, planning, co-ordinating and managing care for those with mental health or neurodevelopmental needs • Experience of assessing and managing complex needs including risk within a structured framework such as CPA/Safeguarding Children frameworks • Experience of effectively managing mental health related issues/referrals/need for advice face to face and virtually, including telephone, at times of high demand and in relation to highly stressful/emotive situations. • Experience of making decisions autonomously whilst working with people and families in the community setting Desirable • Experience of working in a crisis/intensive support model • Experience in CAMHS although the lack of explicit CAMHS experience is not an excluding factor • Experience of working with people who may have neurodevelopmental needs such as Autism, ADHD or Learning Disability Skills, Knowledge and Abilities Essential • Knowledge of Child Social Care and Mental Health legislation and safeguarding, relevant NICE guidance and professional Code of Conduct • Understanding of the principles of the Care Programme Approach and Community Care Act • Knowledge of the wide range of statutory independent and third sector service provision for CYP • Skills of assessing, planning and evaluating care, for people with mental health related difficulties, utilising recognised models of care • Ability to complete and interpret Clinical Outcome measures such as CORC • Skilled in interventions to engage those who may otherwise not access services when in need Knowledge of Children's Act, Mental Health Act, the Mental Capacity Act and DOLS • Ability to support and supervise the performance, practice of less experienced and support level staff as appropriate, • Ability to work flexibly (as part of a rota) to meet the extended remit of the service (8am - 10pm, 365 days a year • Proven ability to manage own caseload and work as part of a multi-disciplinary team • Skills and knowledge to determine when and how to mobilise appropriate professionals in the event a health, mental health or social emergency • Skilled in the assessment and formulation of clinical risk and developing and communicating appropriate risk management plans • Supervisory skills and knowledge to be able to provide clinical supervision Desirable • Understanding of psychological formulation • Skilled in utilising recognised profession specific assessments Personal Attributes Essential • Leadership skills, ability to motivate and influence others • Exemplary personal standards of conduct and behaviour • Possesses values and beliefs that enables others to deliver services effectively • Passion for working with C& YP experiencing mental health difficulties • Ability to move between trusts sites Employer details Employer name Midlands Partnership NHS Foundation Trust Address Block 6, St Michael's Court Trent Valley Road Lichfield WS13 6EE Any attachments will be accessible after you click to apply. 301-VA-26-7716632
