Outreach Coordinator Overview: Mobile Physician Services (MPS) is seeking a full-time Outreach Coordinator to support the continued growth and operational success of our CMS GUIDE Model and broader business development initiatives. This role is integral to coordinating outreach efforts, scheduling activities, and follow-up workflows across the Business Development, Intake, and operational teams. This individual will work closely with the Business Development team, Intake, and leadership to support GUIDE outreach, provider coordination, and network expansion activities critical to program success. To excel in this role, you should be detail-oriented, proactive, and comfortable managing multiple workstreams simultaneously. Strong communication skills, follow-through, and familiarity with healthcare workflows are essential. Essential Functions Include: Coordinating all GUIDE outreach activities, including scheduling meetings and follow-ups for the Business Development team. Managing outbound outreach scheduling for the Business Development team across assigned markets. Tracking and following up on pending New Patient Packets (NPPs) in coordination with Intake, including missing items, patient or POA outreach, and status updates until completion. Conducting targeted primary care provider (PCP) outreach in strategic geographic areas. Supporting respite network expansion through outbound calls and relationship development with community partners. Assisting with medical records coordination related to GUIDE patients, including requesting, tracking, and ensuring timely receipt of records. Maintaining accurate documentation of outreach, scheduling, and follow-up activities across internal systems. Serving as a knowledgeable representative of Mobile Physician Services, able to speak to the organization’s mission, services, and GUIDE Model offering. Contributing to team efforts by accomplishing related administrative and operational tasks as needed. Interested Candidates Should Have: Minimum High School Diploma or equivalent required (Associate’s or Bachelor’s degree preferred). 6 months to 1 year of experience in healthcare outreach, intake coordination, business development support, or administrative roles. Strong organizational, communication, and time-management skills. Ability to manage multiple follow-up workflows simultaneously. Computer literacy, including proficiency with Microsoft Office products. Prior healthcare, medical office, or provider-facing experience preferred. Work Environment: This is a hybrid outreach and coordination role, combining administrative responsibilities with outbound calls and cross-department collaboration.
