At Resolution Care Services, we are looking for a compassionate and organised individual to join our team in a combined Care Coordinator / Senior Carer role. This position offers the opportunity to support the coordination of high-quality care services while continuing to provide direct support to individuals in their own homes. The Role This role combines care coordination responsibilities (28 hours) with hands-on care \& support delivery (12 hours). You will help ensure our services run smoothly while supporting individuals to live safely, comfortably, and independently in their own homes. Key Responsibilities * Coordinate and maintain weekly care schedules. * Support the smooth day-to-day running of the service. * Visit new individuals and support initial assessments and care planning. * Liaise with individuals and families to review care needs. * Provide support and guidance to care staff. * Deliver hands-on care and support where required. * Maintain accurate records using electronic care systems. * Participate in the on-call rota as part of the senior team. What We Are Looking For * Experience as a Senior Carer or Care Coordinator within domiciliary care. * Experience providing hands-on care in the community. * Strong organisational and communication skills. * Compassionate, reliable, and committed to person-centred care. * Comfortable using electronic care systems. * Full UK driving licence and access to a vehicle. Desirable * Experience completing initial assessments or care reviews. * Experience with rota planning or scheduling. * Health and Social Care qualification.
