We are currently seeking a Care Plan Coordinator. Job Responsibilities and Duties: * Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines. * Develop and implement individualized care plans in collaboration with interdisciplinary teams. * Perform utilization reviews to ensure appropriate resource allocation and adherence to Medicare regulations. * Maintain accurate medical documentation and records in EHR systems, ensuring HIPAA compliance. * Provide guidance and support to nursing staff regarding patient care and documentation practices.
